欧州における発展

中世から近世にかけてのヨーロッパで基礎が築かれ、特にドイツの教育改革が現代的な研究大学のモデルに大きな影響を与えました。イギリスでは19世紀後半に研究学位が登場し、1860年にはロンドン大学が科学博士（DSc）を導入しましたが、当初は高度な学習コースとしての性格が強く、現代的な「研究学位」としてのDScは1882年のダラム大学の導入が先駆けとなりました。

北米における展開

アメリカ合衆国では、ドイツの学術モデルを取り入れる形でPhD制度が発展しました。現在では、研究中心の大学（Research I Universitiesなど）を中心に、厳格な要件と長い修業期間を伴うプログラムが提供されています。

その他の地域的な差異

中国では、哲学博士のほか、経済学、法学、教育学、文学、歴史学、理学、工学、農学、医学、軍事学、経営学といった分野別に博士号が細分化されています。また、ロシアや旧ソ連圏では、伝統的に「科学候補（Candidate of Sciences）」と、より上位の「科学博士（Doctor of Sciences）」という二段階の構造が存在していました。

学位の要件と授与の形態

PhDの取得には、通常、指導教授の監督下での研究と、その成果をまとめた論文（ディザテーション）の提出、および口頭試問（ディフェンス）への合格が必要です。一部の国や大学では、個別の論文ではなく、既に出版された複数の学術論文をまとめる「論文集による博士号（PhD by publication）」という形態も認められています。

また、学位の格付けによってガウンなどの正装が異なる場合があります。例えばケンブリッジ大学では、PhDは「下位博士号」と位置づけられており、上位博士号の真紅のガウンではなく、真紅の面（facing）がついた黒い修士用ガウンを着用します。
